Small bleeding during sex?

So..my boyfriend and I had sex the other day and there was a very small spot of blood on the bed when we were done. I am on the pill but I have been on the same pill for 4 years. I have also been with the same partner for 4 years. This has never happened before and I just had a check up 4 months ago and was fine. Has any one had this happen to them or does any one know why this might be happening?

If it was small maybe he just injured you accidentially. Like a little tear or something.
